Are there any circumstances, within or outside trading halts when options may not be exercised and be allowed to lapse even if they are in-the-money?

The OCC will stop automatic exercise of options whose underlying are subject to a long-term trading halt. However, you may still exercise these options by instructing your broker to do so, and the OCC will process them as usual.
